Serenity Mental Health Service, PLLC takes safeguarding our clients' information extremely seriously. Serenity Mental Health Services is bound by law to protect client confidentiality. Any disclosure of your treatment to others will require your explicit written consent. However, our providers are mandated reporters which may require the disclosure of certain information without your consent, including, indications of possible child or elder abuse, or planned bodily harm to oneself or to others. Providers are legally bound to report these to the appropriate authorities. Clients are required to sign a statement of understanding of their rights to privacy, which will be provided via the Patient Portal.
Due to provider licensing requirements, you must be a resident of the state of Texas in order to receive services through Serenity Mental Health Services, PLLC. A state-issued photo I.D. is required for services. If you do not not have one at the time of our appointment, your appointment will either have to be rescheduled or cancelled.
All services are conducted virtually through a HIPAA-compliant platform. You will need to have access to a smart device or computer with a working camera and microphone. You will also need to sign the telehealth consent form which will be sent to you in your intake packet which you will receive once your appointment is confirmed.
Please note, intake forms must be completed at least 24 hours before your appointment, or your appointment may be subject to cancellation.
No-shows and Late Cancellations will be charged a fee of $75 for the appointment. Appointments canceled less than 24 hours in advance are considered a no show. It is the responsibility of the client to notify the provider of a cancellation at least 24 hours in advance of the scheduled appointment to avoid the no show fee. Clients who are more than 10 minutes late will also be considered a no-show due to leaving inadequate time for treatment services.
We require that clients make full payment at the time of services. You will be provided with information about the cost of services prior to scheduling your appointment. We can also provide a superbill for reimbursement from your insurance company.
Please not that a credit card must be on file in order to receive services.
Safety and good mental health are our top priority. For this reason, all controlled substances are used very conservatively at Serenity Mental Health Services, PLLC and will be done in accordance with a strict treatment plan developed together between provider and client. Please note that the prescription of any benzodiazepines, such as Xanax, Valium, Klonopin, and Ativan, will be done only on a time-limited basis and in restricted amounts. Prescriptions for stimulants will require a signed Stimulant Agreement Form which will be fully disclosed and discussed with you by your provider at the time of your appointment.

Serenity Mental Health Services, PLLC reserves the right to terminate services if at any time services are no longer beneficial to the client, when the client may be in need of services not provided by our clinic, in the event of repeat nonpayment for services or missed appointments, when the client is noncompliant with the developed treatment plan, or if the client or other support person become hostile or aggressive toward the provider. Please note that advance notice will be given prior to the termination of services so that the client and provider can try to reach a mutually beneficial agreement. If they are unable to do so, then services will be terminated.
Serenity Mental Health Services, PLLC is committed to ensuring that everyone has access to mental health care. Our services are provided by appointment only during our regular business hours of Monday through Friday 9am to 5 pm, however, we have a lot of flexibility and can work with clients under certain circumstances to find an appointment time that works for you, including early mornings/late evenings or weekends. If you need to schedule an appointment outside of our regular business hours, please send us a message so that we can find a date and time that works.
